Business Certification Using Printable Forms—Recertification User Guide

Step 1: Download, print and review all CBE Certification Program forms and information.
Step 2: Review the CBE Frequently Asked Questions and Certification Fact Sheet.
Step 3: Complete the Recertification Application and attach the following documents:**
  • Exhibit 1: Copy of applicant's most recent financial statement (which is less than 120 days old).
  • Exhibit 2: District and federal tax returns for the past two years.
  • Exhibit 3: Current District of Columbia Professional License (i.e., Contractor, Engineer, Architect).

**If applying for recertification as a Disadvantaged Business Enterprise (DBE), you must include the principal owners' personal financial statement (less than 120 days old) and a notarized DBE letter demonstrating the causes that contribute to your economic and social disadvantaged status (i.e., access to capital, credit, bonding, equal opportunity in contracts, educational opportunities and housing.)

Step 4: Submit the CBE Recertification Application and required documentation to:
